If you read the report of the Gulf oil spill, you will find that regulations were part of the cause. The procedure manual for emergencies resembled a novel. It was too big and convoluted to be effective. One of the reasons they didn't divert the oil/methane mix back out over the water was because of fear of environmental fines. I see it in my industry as well. Instead of streamlining and simplifying after a root cause analysis, the tendency is to just pile on more rules.
